We can also put families in touch with trained volunteers who provide in-home respite, equipment loan and transport. We can also arrange clinical, financial and bereavement support where required.

Our Occupational Therapy team assesses people’s ability to carry out their usual activities of daily living and help overcome any difficulties they may be experiencing. People may find it harder to do their usual activities due to the aging process, illness, an accident, surgery, or a disability.
Our Men’s Health Nurse provides generalist nursing services to men of all ages. We also provide a comprehensive clinical nursing service to men living with prostate cancer, who require:
* Community urinary catheter care and education
* Continence assessment and education
* Men’s Health Assessment, coordination and referrals
* Health promotion and education
* Optimising male engagement with health services and health promotion programs

Hospital in the Home enables eligible patients to access hospital services, such as nursing and medical care, in the comfort of their own home.
Our Hospital in the Home team works with patients, families and carers to providing ongoing medical treatment including intravenous antibiotics, pre-surgery treatments, Deep Vein Thrombosis (DVT), blood disorders, wound dressings, sub-acute surgical care for wounds, stomas and drains, physiotherapy services including respiratory and much more.
Our Exercise Physiologist works as part of the Cardiac, Heart Failure and Pulmonary Rehabilitation Services in a multidisciplinary team.
The Exercise Physiologist develops exercise programs which supports patients to manage their chronic conditions and improve their overall quality of life.

Our trained dietitians help people understand the relationship between food and health. Dietitians can help you make dietary choices to support your health, and to prevent and treat illness and disease.
Dieticians can assist with a range of conditions including diabetes, being overweight, coeliac disease, irritable bowel syndrome, raised cholesterol, malnutrition, childhood feeding issues, and tube feeding.

* Information and education for individuals and groups.
Our Diabetes Educator provides diabetes education and support for people with diabetes and their families, either in one to one or group settings. They also work closely with the Diabetes Dietician.
We also provide education to health care professionals and support promotional activities that encourage people with diabetes to maintain a healthy lifestyle.
To access our service, you must be diagnosed with Type 1, Type 2, or Gestational Diabetes, Pre-diabetes, or require diet or insulin management. No referral is required.
Generalist Community Nursing is a free confidential service providing a range of services including post-acute hospital nursing, wound care, continence advice and management, management of venous access devices, Hospital in the Home (HITH), Palliative Care Nursing, nursing assessments, and client referral to other care providers including allied health and the Aged Care Assessment Team.
